This plan covers routine dental exams and cleanings as well as non-routine care, such as oral surgery and crowns.

What to Know

  • You may receive care from any dentist; however, you have greater cost savings when you use a provider in the Anthem PPO network. When you schedule your appointment, confirm the dentist is in-network for Anthem dental.
  • The plan covers most preventative visits at 100%, and for all other services, you will be responsible for a percentage of the cost after your deductible is satisfied and up to the annual maximum.
  • You don’t need a dental ID card to receive care – just give your name, date of birth, and student ID number preceded by V0. For example: John Doe, 01/01/2010, V0123456. If you’d prefer a physical dental ID card, you can print one from the Anthem website or access it on the Sydney app.
  • If you need dental care while traveling outside the U.S.:
    • You may receive regular or emergency dental care from any licensed dentist anywhere in the world and the plan will reimburse you.
    • You will need to pay the dentist for the services rendered at that time.
    • Ask the dentist for a detailed billing statement that includes a clear detailed description of services, date of service, cost of the service, and a signature from the provider.
      • The currency rate will be converted by Anthem using the exchange rate based on the date of service, but it must be noted on the claim what type of currency was used.
    • When you return home, forward the billing statement to Anthem (which must be translated into English before it is submitted), attached to a foreign claims form, and receipt of payment.
