This plan covers routine exams and includes annual allowances for eyeglass frames and lenses or contact lenses.

What to Know

  • You may receive care from an eyecare provider; however, you have greater cost savings when you use a provider in the EyeMed Insight Network.
  • If you use an out-of-network provider, you will pay the full cost up front and submit an out-of-network claim form for reimbursement.
  • The plan features a diverse national network which includes preferred eye care practitioners and partners such as LensCrafters, Sears Optical, and Target Optical.
  • You don’t need a vision ID card to receive care – just give your name and date of birth when you visit a provider. If you’d prefer a physical vision ID card, you can print one on the EyeMed website or access it through the EyeMed members app.
  • If you need eye care while traveling outside the U.S.:
    • You may receive care from any licensed eye doctors anywhere in world and the plan will reimburse you at out-of-network rates.
    • You will need to pay the eye doctor for services rendered at that time.
    • Ask the eye doctor for a detailed billing statement that includes a clear detailed description of services, date of service, cost of service, and a signature from the provider.
    • When you return, forward the billing statement to EyeMed (which must be translated into English before it is submitted), attached to a claims form, and receipt of payment
